A case of catheter-related bloodstream infection caused by Candida guilliermondii that was successfully treated with fosfluconazole

Abstract
In Japan, Candida guilliermondii is rarely identified as a causative organism of candidemia associated with catheter-related bloodstream infection (CR-BSI). We report a case in which treatment with fosfluconazole (F-FLCZ) was clinically effective in a patient with CR-BSI caused by C. guilliermondii. The patient was a woman in her 80s who was hospitalized for disuse syndrome and was receiving nutritional management via a peripherally inserted central catheter (PICC). When she developed fever, an interim report of a blood culture collected during a febrile episode indicated the presence of yeast-like fungi. We removed the PICC and initiated the patient on F-FLCZ treatment. The causative organism was subsequently identified as C. guilliermondii, with an MIC of fluconazole of 16 μg/mL; however, since repeat blood culture confirmed negative conversion, F-FLCZ treatment was completed after a total of 26 days of administration. For our patient with CR-BSI caused by C. guilliermondii, we selected antimicrobial therapy based on the results of a blood culture and achieved clinical cure through treatment with F-FLCZ.
